       Title: "Treatment Planning Utilizing the CT Navigator System"

    The November 19, 2009 Dental Implant Study Club featured Dr. Stanley Rye, a nationally/internationally renowned prosthodontist from Atlanta.  Although he covered many topics, the thrust of his presentation was Implant Restoration using Navigator Technology.  Dr. Rye began by showing slides of failures due to poor implant placement. He discussed the future of CAD – CAM technology and also the Breedan attachments to use with cad-cam bars in minimal vertical dimension cases. Dr. Rye praised the advantage of ENCODE restorations and went over the use of virtual impressions for these fixtures.

 Pearl: Nitrate coatings for implants contain nickel which can cause a reaction in sensitive patients.

 

    Dr. Rye also showed his technique for DIEM dentures and the value of making a duplicate denture to use in the future when repairs became necessary. Much time was devoted to the importance of post-op instructions in preventing failures. The advantages of the quick bridge technique were enumerated and the technique shown. This has made DIEM procedures much simpler.  The Navigator system saves time, eliminates guesswork, and enables collaboration between surgical and restorative teams. This technology allows better treatment plans and better treatment outcomes.

 

Pearl: Have radiologist check CBCT scan for pathology to avoid liability.

    Dr. Rye stressed the need for a minimum 6 implants in the maxilla when placing a fixed restoration.  If restoring back to the second molar, he recommends 8 implants.  He showed a way to duplicate a denture to use as a surgical guide using Lang’s denture duplicator.  This is helpful when doing a DIEM or locator case for the surgeon.

 

Pearl: Use PAM as a separator when duplicating.

 

    Dr. Rye thinks that the cost for navigator technology will continue to decrease and it will become standard treatment for most implant placement in the near future.  The use of this technology has been used by members of this study club for over 18 months.  360 Imaging has been the mainstay for this group in helping with this technology.  It allows the restorative dentist and the surgeon to plan together for the best outcome.  For those looking to get a camera to take intraoral shots, Dr. Rye recommends the Nikon G-90.

       Title: "Provisionalization, ENCODE, and Implant Occlusion"
         
       Dr. Ed Schlissel gave an excellent presentation at the study academy’s first meeting of the 2009-2010 academic year. The reviews for his lecture were very positive and he was a terrific teacher. Among other things, Dr. Schlissel discussed different dental materials and methods in implant restoration. He stressed the importance of remembering and understanding the basic biomechanics of materials to ensure that the restorative dentist makes an educated decision. He discussed modulus of elasticity, fatigue, vector forces, occlusion, etc. that we all studied in dental school but haven’t reviewed much since that time. It was very interesting to note the way the same factors work very differently in implant restorations when compared to the natural dentition.    Dr. Schlissel showed used vector forces to demonstrate why and how lateral forces are transmitted through the implant. He also compared the force distribution of external connection (hex) implants versus internal connection (hex) implants, and explained why we mainly use internal connection implants now.

        Dr. Schlissel also gave the group some numbers and ratios to consider. For instance, a molar has 9 times the force that a central has when in centric occlusion. He also mentioned that, because of the direction of the vector, lateral forces can be as as much as 17 times stronger than vertical forces. He suggested splinting in the maxilla more often than in the mandible, primarily for boney support. Dr. Schlissel also suggested opposing porcelain with resin (hybrid, etc.) when fully restoring both arches, in order to prevent fracture. His philosophy in regards to impressions is that there is always going to be some distortion. For that reason, he prefers the open tray technique as opposed to closed tray so the distortion is minimized. He also likes optical impressions for the same reason: lack of distortion upon stretching of the material. At this point Dr. Schlissel discussed the ENCODE system.

        Dr. Schlissel touched briefly on different occlusal philosophies, and was careful not step on any toes! He basically discussed Dawson concepts, but he was big on flat plane teeth and suggested modeling implant crown occlusion to avoid lateral forces. If unavoidable, he suggested trying to group the function in order to minimize the lateral vector. A complete upper denture with a “flat” palatal vault that opposes lower implants will be LESS STABLE than when it opposes a complete lower denture without implants. This is something to watch out for and predict before it happens!

        Dr. Schlissel also reviewed the importance of implant width in minimizing a “cantilever effect”.  Simply put: the wider the implant, the better…With a 4.0 diameter implant as a baseline -                                                                                                                         

                      A 5.0 implant will have 20% less force on the screw.                                                                                

                      A 6.0 implant will have 34% less force on the screw.

 

He also pointed out that the implant “screw” is not actually a screw, but rather a bolt. By definition, a “screw” creates friction by displacing the medium in which it is placed. The term “bolt” describes a tensile force in which the bolt is actually “stretched” creating tension between the parts. In an internal connection implant, force is displaced in the connection itself, the “bolt” just holds it together. By contrast, the screws or “bolts” in external connection implants have to withstand much of the force. For this reason, they are much better used in multiple implant situations when they can be splinted together to stop rotational forces and stress on the screw.

 

Dr. Schlissel also encouraged “squaring” or “flattening” the teeth adjacent to implant crowns to create wider flat, broad, contacts as a way of opposing rotational force. He gave a visual example of the way the adjacent teeth and the broad contacts on the implant crown act to counter the rotational forces: Think of square blocks tightly positioned next to each other and then trying to turn one of them, versus a round peg between square blocks that can be easily rotated.

Dr. Schlissel also encourages creating longer contacts for an implant crown and, as Dennis Tarnnow has demonstrated in his research, making the contact 5mm or less from the height of bone on the adjacent teeth. If the contact is less than 5mm from the height of the bone, there is a 95% chance of regenerating a decent papilla. When you go over 5mm, the odds of developing papilla drop quickly. Sometimes adjacent teeth may need a veneer or other restoration to allow the contact to be reshaped closer to the gingival. Especially if the adjacent teeth are triangular in shape, this may be necessary in order to stay within the 5mm range and avoid the dreaded black triangle!

            The annual end-of-year trip took the group to the Bahamas this year for both members and their spouses. With stops in both Nassau and Freeport, the Carnival “Fascination” housed the final lecture series of the academic year. Sponsored by Northeast Oral Surgery, the cruise was the perfect send-off to the year’s hard work. In addition to the onboard fun, the group ventured off the ship to the Atlantis resort for a day of fun in the sun, and also took advantage of the shopping opportunities in Nassau and Freeport.

            The lecture presented by Dr. Donald Purcell was broken into two afternoon sessions. Dr. Purcell gave an overview of implant treatment planning, and he presented a series of complex implant restorative cases. He also touched on the team approach to treatment planning as it involves restorative dentists, surgical specialists, and oftentimes orthodontists.

            In the last regular meeting of the academic year, Dr. Ritter gave an informative presentation regarding cosmetic restorations that did not include dental implants. He shared with the group a wealth of information related to various products, manufacturers, and the successes & failures he has had with various materials.

            Dr. Ritter presented a number of his own cases that included cosmetic work (i.e. veneers, pressed ceramics, etc.) as well as splint therapy for TMJ patients. He discussed color matching philosophies as they related to cosmetic restorations, and he also discussed occlusal philosophies as they related to splint therapy.

            For the March “mini-residency” meetings, the presentation sequence was reversed. Group members presented their own cases on Thursday night, and Dr. Smith presented his own lecture afterwards. Cases were presented by Dr. Basquill, Dr. Carson, Dr. Feuerstein and Dr. Koutroulakis. Both the restorative dentists and surgeons participated in the case presentations.

            After the group presentations on Thursday, Dr. Smith gave his lecture on edentulous restorations. Friday morning he discussed hybrid prosthetics as well as mechanical complications that restoring dentists may encounter. Dr. Smith closed the Friday meeting by thanking for the group for their participation in the mini-residency lecture series.

            Moving forward in the “mini-residency” series, Dr. Smith discussed provisional implant restorations, impressions and the aesthetic concerns that come with them. He introduced a great deal of his own methodology, and he also shared specific supplies and products that he has had success with in his own prosthodontic practice.

            While the Thursday evening meeting was primarily guided by Dr. Smith and involved cases that he has restored, the Friday morning meeting consisted of case presentations by group members. Cases were presented by Dr. Basquill, Dr. Koenig, Dr. Koutroulakis and Dr. Davant, Dr. Moss, and Dr. Richardson. Presentations were done by the restorative dentists and surgeons together.

            Topic: “Restoring the Edentulous Maxilla”

 

            Dr. Martel presented a series of cases in which edentulous maxillas were restored. He discussed the treatment planning and mechanical differences between mandibular and maxillary full arch restorations. There was a great deal of group discussion regarding full mouth restorations.

            Dr. Martel also touched on the importance of presenting treatment plans that give patients realistic expectations. He discussed the tendency of patients to make unrealistic assumptions about their care, and how important it is for the dentist to walk the patient through the proposed treatment in a way that will prepare them for the outcome that they can realistically expect. Dr. Martel emphasized that it is the doctor’s responsibility to ensure that the patient’s needs are met, and the doctor must have a good understanding of the patient’s needs and expectations in order to effectively introduce a treatment plan and its results to the patient.

            In the second “mini-residency” lecture of the academic year, Dr. Smith built on the concepts he introduced at last month’s meeting. He discussed initial treatment planning in restorative cases, including treatment sequence and aesthetic considerations that the dentist must make.

            Dr. Smith presented many of his own cases as he walked the group through his treatment planning process. He also engaged the group in discussions of their own cases. Beginning with the next meeting, members will bring case presentations of patients they are currently treating in their own practices.

            Dr. Ridgell gave a very informative lecture on immediate mandibular restorations or DIEM cases. He discussed the new QuickBridge technology as well as treatment planning, timing and coordinating between the surgeon’s office and the restorative dentist’s office.

            In addition to sharing the new QuickBridge technique, Dr. Ridgell presented a number of his own cases. Dr. Ridgell asked how many of the group members have done DIEM cases with our surgeons, and several responded. The group discussed their experiences with DIEM cases and, in particular, the way CT scans improved the treatment planning process for them.

            In the first of a series of Implant Restoration Mini-Residency lectures planned for the 2008-2009 academic year, Dr. Jay Smith delivered an informative presentation on treatment planning. He discussed the benefit and value of dental implant treatment and encouraged the group to re-assess the ways in which they talk to patients about implants. He discussed standard of care, long-term versus short-term costs, longevity of the restoration, etc.

            In addition to discussing how to improve your case and fee presentations, Dr. Smith touched on the initial considerations each dentist should make when formulating an implant treatment plan.

            Ms. Garbo is a motivational speaker and dental practice consultant. As the keynote speaker for the first SC Academy of Advanced Implant Studies meeting of the 2008-2009 academic year, Ms. Garbo delivered an inspiring presentation to both the doctors and their teams. The lecture encouraged doctors, assistants, hygienists and administrative to rethink their customer service approaches in working with patients. Ms. Garbo inspired attendees to approach their jobs with attitudes of empathy, encouragement and caring.

            The kick-off meeting was a tremendous success. In addition to Ms. Garbo’s moving presentation, thousands of dollars in prizes were raffled. The prizes given away included cash, two pearl necklaces and matching earrings, a Garmin GPS, Biomet 3i merchandise credit, and dozens of gift certificates to various restaurants, stores and spas.

            Mr. Twigg presented a synopsis of employment law concerns and the importance of being in compliance with both local and federal employment laws. He discussed a number of ways dentists can protect their practices from frivolous employment related lawsuits and also the ways that employment policies and handbook can boost office morale and improve team building in the dental practice.

            Mr. Twigg also reviewed the employment compliance services provided by Bent Ericksen and Associates and their local practice coordinator, Evelyn Horne, with Dental 360. These services include the HR Director software package as well as technical support from Bent Ericksen and local Dental 360 representatives.

            Mr. Groover is the owner and head technician at Dental Laboratory Associates in Savannah, GA. Each night his lecture included a number of cases DLA has worked on, and he also walked the group through the steps the lab takes in the restoration of each case. The difficulty of the restorations presented each night varied.

            Mr. Groover discussed in detail the way the his lab has incorporated ENCODE technology, and he also provided models, parts and pieces to help facilitate the discussion. Attendees were encouraged to bring current cases of their own to discuss treatment planning options. After Mr. Groover’s formal lecture, the group discussed a number of their own cases and treatment plans. Mr. Groover answered a number of questions related to the role of the lab technician in implant cases, and he also discussed some of DLA’s fees and services.

            Dr. Blackwell delivered an informative presentation that touched on several of today’s “hot topics” in implant dentistry. In particular, Dr. Blackwell presented a wealth of research and information related to platform switching. Dr. Blackwell discussed multiple studies outlined in current literature that indicate the effectiveness of platform switching in reducing the loss of bone around dental implants to the first thread of the implant. In addition to his presentation of current topics in implant dentistry, Dr. Blackwell answered numerous questions posed by the group related to their own restorative implant cases.
